South Windsor Transition Program (SWTP)
Program Overview
The SWTP is South Windsor’s public transition program and is currently located in the South Windsor High School Annex. SWTP serves a small cohort of young adult learners, aged 18-22, who are determined through the PPT process to need additional support in the areas of post-secondary planning, vocational training, and life skills development. Our program philosophy focuses on individualized learning, community engagement and person-centered planning.

- A smoother transition to adult services, supports, and/or post-secondary educational experiences

- Resume development and interview practice
Independent Living Skills
- Money management and budgeting

- Transportation and travel training
- Health, safety, and self-advocacy skills
Community Participation
- Community outings and functional learning experiences
- Social skills instruction in real-world settings
- Recreational and leisure activities
- Volunteering and civic engagement
